Subject: Termination of services of Sharma Nursing Home from CGHS empanelment.

This has reference to your letter dated 14.10.2025, wherein it has been intimated that due to unforeseen circumstances, your institution will not be able to continue providing services to CGHS beneficiaries and has requested for voluntary withdrawal from CGHS empanelment.

In view of your request and as per the terms and conditions laid down in the Memorandum of Agreement (MoA), the withdrawal has been accepted. Accordingly, the empanelment of Sharma Nursing Home under CGHS stands terminated with effect from 14.11.2025 after completion of the one-month notice period.

CGHS rates & terms shall continue to apply to any CGHS beneficiary under it at the HCO until their discharge.

Further, the process for release of Performance Bank Guarantee (PBG) shall be initiated in accordance with the provisions of the MoA.

If you are a CGHS beneficiary currently undergoing treatment at Sharma Nursing Home, you should be aware of the following:

• The nursing home will no longer be part of the CGHS empaneled network after 14 November 2025
